알코올 중독자의 심리학적 발생 원인과 대처방안에 관한 연구

Abstract

Recently in the world, the seriousness of alcohol addiction has been rising, so that each country is implementing policies for controlling the alcohol use in the national level. As an impulse to use alcohol without considering negative or harmful results, the alcohol addiction means the physical/psychological dependence on alcohol. It is to habitually drink with a continuous desire through complex causes such as genetic, biological, psychological, familial, and cultural causes, so that body and mind cannot have normal functions. W.H.O(World Health Organization) defines alcoholism as "the case of drinking beyond the range of traditional drinking habits, or beyond the range of drinking habits in society". According to the report by the Supreme Prosecutors' Office in 2012, 4.6% of all offenders reported alcohol use on the day of the incident and 8.5% of violent crimes (assault, fire, rape, murder) occurred under alcohol use(Jeong, Jin-Wook, 2015). The cost of treatment for alcoholism is steadily increasing. The number of people with alcohol mental-behavior disorders increased from 72,938 in 2003 to 108,340 in 2010 67% increase. In addition, the cost of alcohol treatment for psychiatric disorders increased 2.3 times from 118,346 billion in 2003 to 274,517 billion in 2010. Alcohol addiction is a risk factor for various physical and mental diseases. Alcohol use disorder patients have high prevalence of coexistence with psychiatric diseases such as liver diseases, brain damage, peripheral neuropathy, cardiovascular diseases, tumors and other physical diseases and anxiety and depression. Even though the social problem by alcoholics is generally in the serious state, there are not many researches on the psychological causes for alcohol addiction regarding the initial causes related to the current status of high-risk group and prevalence of alcohol addiction in Korea. Even though the mental health project plan of the government presents the alcohol addiction management system, the implementation is insufficient. There were announcements of the national alcohol measures 'Blue Bird Plan' in 2006 and 2010 while there were neither follow-up measures nor national-level governance for the comprehensive and systematic management. Due to such complex causes and socio-environmental changes, the number of alcoholics is continuously increasing.

Based on the results of analysis, this study makes suggestions as follows. First, in the results of mainly analyzing the psychological causes closely related to alcohol addiction out of many causes, it would be necessary to emphasize the importance of alcoholics' respect for themselves, respect and conversation between family members, adolescents' smooth communication with parents, and peer relationships, and also to provide the thorough preventive education to make them perceive the seriousness of harmful effects of drinking. And it would be essential for all the family members to quickly react, to cognize the harmfulness, and to learn about illnesses even before having an alcoholic. Second, as the youth generations and the early middle-aged generations, the age group in 30s-60s knows a certain degree of harmfulness of alcohol, and liver/skin illnesses that could be caused by alcohol through internet or the media. Also, the actual status should be observed by visiting the treatment facilities for alcoholics and also providing thorough education to relevant institutes, universities, and office workers, targeting high school graduates and university graduates, and high-income group and low-income group. The government, local governments, the Ministry of Health and Welfare, Community Addiction Management Centers, and Korean Association for Psychological Rehabilitation should establish the coping measures and actively support the prevention and efficient healing/treatment of alcoholics. Third, the Ministry of Education should establish/implement policies suitable for temporal/environmental changes for the efficient measures such as concert for adolescents, beneficial theater, sports facilities, and adolescent welfare center through thorough education and alternatives to approach alcohol regarding the harmfulness and illnesses of alcohol for adolescents starting from elementary/middle/high schools, and remove the causes in advance through events in the working institutes of office workers in their 30s-60s. And the district offices, county offices, and community service centers should provide the specialized education to general alcohol users, alcoholics, non-alcoholics, and victims of domestic violence and child abuse by addiction in each quarter. Also, the National Health Insurance Corporation should take an action to provide treatments for free by establishing the cooperative system with hospitals to combine the isolation, control, and psychotherapy in accordance with the severity of addiction. The Ministry of Health and Welfare should present the practical policy measures for blocking the alcohol use by visiting alcoholics with the use of social workers and health nurses.| 최근 세계적으로 알코올 중독의 심각성이 대두되어 국가마다 음주사용통제 정책을 국가 차원에서 실시하고 있다. 알코올 중독은 부정적인 결과에 따른 고려를 전혀 하지 않고, 충동적으로 사용하는 신체적․심리적 의존을 말하며 유전적, 생물학적, 심리학적, 가족적, 문화적 등 복합적 원인을 통해 지속적인 욕구를 가지고 심신이 정상적인 기능을 할 수 없도록 상습적으로 음주하는 것을 말한다. W.H.O(세계보건기구)에서는 알코올 중독을 "전통적인 음주습관 범위를 넘거나 또는 사회 전체 음주습관의 영역을 넘어 음주하는 경우"라고 정의하였다. 대검찰청 2012년 산출 결과 보고에 의하면 주취 범죄율은 전체 범죄자 가운데 4.6%가 사건 발생 당일 알코올 사용 상태였으며 강력범죄(폭행, 방화, 강간, 살인)의 8.5%가 알코올 사용 상태에서 일어난다고 하였다(정진욱, 2015). 알코올 중독 치료를 위한 진료비는 꾸준한 증가 추세이다. 알코올 정신행동장애의 치료인원은 2003년 72,938명에서 2010년 108,340명으로 67% 증가하였다. 또한 알코올 정신행동장애 진료비는 2003년 118,346억 원에서 2010년 274,517억 원으로 2.3배 증가하였다. 중독은 다양한 신체적, 정신적 질환 위험요인으로 알코올 사용 장애 환자들에게는 간 질환, 뇌 손상, 말초 신경병증, 심혈관계질환, 종양 등의 신체적 질환과 불안 및 우울증 등 정신과 질환과의 공존 유병률이 높다.

알코올 중독 실태에 관한 많은 연구 발표가 있지만, 최초 발생 원인에서 심리학적 접근에 의한 발생원인에 대한 연구는 미미한 실정이다. 정부의 정신보건 사업계획서에 알코올 중독관리체계가 제시되어 있으나 그 시행은 미흡하고 알코올 중독과 관련하여 국가 알코올 종합대책인 ‘파랑새플랜’ 2006년, 2010년 대책 발표가 있었으나 그 후속 대책을 마련하지 않고 있으며 체계적이고 통합적으로 관리할 수 있는 국가 차원의 거버넌스도 존재하지 않는 실정이다. 이러한 복합적 원인과 사회 환경의 변화에 따라 지속해서 알코올 중독자는 증가하고 있다.

이에 대한 연구 분석의 제언을 제시한다. 첫째, 알코올 접근에 대해 본 연구에서는 여러 원인 중 알코올 중독에 밀접한 관련이 있는 심리학적 발생원인을 중심으로 분석하여 그 대안에 우선으로 대처하면서 중독자 개인이 자신을 존중하는 마음, 가족 간 존경과 대화, 청소년기 부모와 원활한 의사소통, 또래와의 관계가 중요하며 음주 폐해의 심각성을 인식할 수 있는 철저한 예방 교육이 필요하다. 그리고 가정구성원 모두 알코올 중독자 발생 전부터 신속한 대처와 유해성 인식, 질병에 대한 학습도 필수적이다.

둘째, 30~60세의 연령층은 청년세대와 초기 중년 세대로 어느 정도 알코올에 대한 유해성과 질병으로 나타날 수 있는 간 질환, 피부질환 등을 인터넷 또는 언론매체를 통해 알고 있다. 또한 고등학교, 대학교 교육수준이 높은 사용자와 직장, 고소득자 집단과 저소득자 집단을 상대로 관련 기관 소속, 대학교, 직장인 상대로 철저한 교육과 중독자 치료 방문 등을 견학하여 실태를 보도록 해야 할 것이다. 정부와 지방자치 단체, 보건복지부 예산 지원과 중독관리통합지원센터, 한국정신사회재활협회 등에서 예방은 물론 효율적인 중독자 치유, 치료에 적극적으로 지원하고 대처방안도 마련되어야 한다.

셋째, 청소년들은 초․중․고 재학 때부터 알코올에 대한 유해성과 질병에 대하여 교육부에서는 철저한 교육과 알코올 접근 대안으로 청소년을 위한 음악회, 유익한 영화관, 체육시설, 청소년복지센터 등 효율적인 방안을 시대 환경적 변화에 맞추어 정책을 수립 시행하고, 30~60대 직장의 소속기관에서 행사를 통해 원인을 사전 제거하며 일반인 알코올 사용자 및 중독자 상대로 구청, 군청, 주민센터 등에서 분기별로 알코올 비 중독자, 중독에 의한 가정폭력, 아동 학대 예상 대상자 상대로 전문가 교육을 하여야 한다.

또한 알코올 중독자 상대로 병원치료와 약물치료에 대하여 건강보험공단에서는 중독의 심각도에 따라 격리와 통제를 하면서 정신치료와 병행할 수 있도록 병원과 협력 체제를 구축하여 무상으로 치료하도록 조치가 필요하며 보건복지부에서는 사회복지사 및 보건 간호사를 통해 주거지를 방문하여 중독자를 돕는 등 알코올 사용 차단에 실질적인 정책 방안이 제시되어야 한다.
