韓國保健增進 政策의 評價에 관한 硏究

Abstract

After 1980s, the quality of life of the Korean people has been raised mainly by the rise of national income. The average life expectancy of the koreans has been increased 9.88 years to 74.39 years in this period. The disease type has also changed to chronic degenerative disease. The function of tile community health center has been upgraded by the revision of health-related laws in 1995. The public health center have achieved the position of public health organization especially after the IMF bail out of the Korean economy in november 1997. The purpose of this study is to suggest policy alternatives for the development of the Korean community health center in the 21st century. This thesis evaluates the government's health promotion policy in terms of the Gilbert and Specht model. The Gilbert and Specht model consists of four areas of allocation, provision, finance and delivery system of social welfare policy. In Chapter 2, this thesis analyzes the history of the functions of the community health center. The problem of the current public health system in Korea has been characterized in this chapter. Firstly, it is studied on the requirements for receiving the service of medical care front the community health center. The problem of the field practice in the center has also been analyzed. Secondly, It is analyzed current situation and problems on the various programs for the public. It is also studied for the management of acute and chronic infections, family health care program, health and medical policy, chronic degenerative disease management program, mental health and the health care for the old. Thirdly, finance area has been analyzed. The public health program undergoes shortage of budget every year. Furthermore, after the institution of the full-f]edged local government system in 1992, it is evaluated that the size of the budget in local districts depends on the leadership of the head of the community health center. It is desirable that the differences on the supporting fund from the upper government to the locals should be lessened. The national policy is needed to strengthen the function of regional community health center. Fourthly, it is analyzed for only four types of functions since there are various kinds of service delivery system, payment system and disease transit system. The current situation and problems of the repre sentative types are analyzed in Chapter 3. in Chapter 4, preceding studies on the organizational problems are summarized and policy alternatives on the four areas of are suggested. Concludingly, this thesis suggests some ways of being a successful community health center in the new age. The expansion of the number of service recipients, steady developments of the health programs, the more stable financing on the center and the desirable change of the current delivery system are needed. With effective implementation of the suggested policies in this thesis, the community health center as the real public health care organization can function for the local dwellers.
